Rewrote HAProxy Virtual Hosts dashboard to use KissTheme:
- Self-contained inline CSS using KISS variables
- Removed external dashboard.css dependency
- Add vhost form with domain/backend/SSL inputs
- Vhosts table with status badges and actions
- Edit modal and delete confirmation dialogs
- Toast notifications for user feedback
